Use this form to request your Baptism Certificate (if baptized at SVdP only) or Confirmation Certificate (if confirmed at SVdP).

- Request must be accompanied with a copy of birth certificate; if the certificate is for your child or if you have changed your name.

- All adults requesting a Baptismial Certificate whether for themselves or for their child must submit a copy of a valid driver's license.

Please give three business days to process your request.
